I presume you can use the questionnaires on
1) How was the training conducted
2) Questions on Trainers (trainers feedback, trainers impact on
participants)
3) Training effectiveness
4) Training needs identified (process & weightage, employee's feedback)
5) Employee's feedback on training sessions
6) Employee's feedback on bringing out changes in training & development
activities
Hope this has given you some clarity on how to prepare the questionnaire and the base to formaulate the same.....
