I presume you can use the questionnaires on:
1) How the training was conducted
2) Questions on Trainers (trainer's feedback, trainer's impact on participants)
3) Training effectiveness
4) Training needs identified (process & weightage, employees' feedback)
5) Employees' feedback on training sessions
6) Employees' feedback on bringing about changes in training and development activities
