and while interviewing for senior and middle level persons, try to prepare a interview questionnaire and in a confident way just ask them those questions, (but donot behave them like a taking interview to fresher).

I have a question on this: I noticed that the feedback form asked for name and department. In my experience in the USA and here in the UAE, that info has been optional with the intent to get more objective feedback without concern for being identified for the content.
Is it common in India to request your name etc? Why?
