Unless you throw more light on your company information, its employee strength, orgn. structure etc
it will be difficult to opine on this. Regarding performance review form, you may look on the right side of your screen, where it is available.

Performance Management System or PMS in short, is an organisational philosophy. Those organisations which wish to improve their review and also improve the process of differentiating between horses and donkes, follow the PMS route. Performance appraisal does not cover discipline, attendance, safety procedures. It is basically on by doing a particular work, assigned to any employee, what will be the ultimate result. Directly or indirectly added to the bottomline or turnover or profit. You should know what can be measured and how they can be measured . In other words, you should have, for each job role, the Key Result Areas measured by Key Performance Indicators. The organisations management should spell out clearly how they want to increase the revenue and how much. This is shared with each department and each employee. The immediate superior sits with each employee and determine the goals for the subsequent year depending upon the job role. the Key Result Area could be revenue growth, customer growth, sales, saving, expansion, customer service, numbers per day or per shift in terms of production. Three areas are mainly focused for Performance - Profit, Process and People. At senior level the KRAs could be people development, talent identification and nurturing. At middle level the focus could be more on Process part of it and at the bottom could be production or productivity or profit... Am a HR Consultant, am a Trainer on various HR initiatives and I have audited and improved the PMS in several organisations in India and gulf countries. The above points came to my mind randomly and hence I have written in a flow. There should be a proper training from top to bottom to understand this philosophy and implement sincerely throughout the organisation from top to bottom, with commitment for that to succeed. If interested, revert to me and I can help your organisation to a great extent. Best wishes
