I hope you are referring to closure of CUG Mobile number issued to employee on behalf of the Company. There is no standard format for such letters. Like the way you posted this question in this forum you can write a simple letter to the service asking them to deactivate services to the said mobile number. Please refer the template below and edit as per your requirement.
