One employee worked in an organization for more than 5 years and joined another organization after proper relieving. Here, the new organization created a new UAN without asking for the old UAN. Due to some personnel issues, the employee left this organization after 2 months. Hence, the employee is not able to claim the old PF amount. As per the suggestion of the PRO, the employee transferred the old PF amount to the new UAN. Now, the employee is not able to link the PAN number with UAN due to a small change in the father's name.

My query is - will the organization seal and sign the Joint Declaration between the employee and employer? Otherwise, suggest to me how to close this PF settlement?

Yes Employer should put seal and sign, as employee has worked there. Please contact your ex employer.
