I would like to know about the appraisal system for the IT Company, procedures with the detailed hierarchy for self-appraisal, Team Lead Appraisal & with the Management appraisal, also would be great if someone could share me with the document related to appraisal with the criteria's weightage pertaining to each performance appraisal & also let me know whether appraisal as to be taken on the monthly, quarterly or yearly basis. What all kind of appraisal can an IT company have?

Dear Parvati,
Rather than concentrating on just Performance Appraisal (PA), I recommend you instituting comprehensive Performance Management System (PMS). PA is part of PMS.
Setting up Performance Management System (PMS) is far more serious activity. It has to be handled with due sensitivity. To institute PMS it requires great exposure across the spectrum of industries. If mishandled it may do more harm than good.
