Dear Shirly
I wonder why you have not identified your CiteHR ID but given your email ID! What is the purpose of asking details of responders? Asking all those at the outset puts people off answering the questionnaire, unless you clearly state how you will use that information.
Kindly digest the info at https://www.analyticsinhr.com/blog/w...-hr-analytics/ and tell us what is the aim of your project and what are it objectives.
In case you have already formulated the aim and objectives, kindly post it.

The initial identification questions will put off people who would not like to make all personal information public.
Purpose of survey, how info will be used needs to be clarified.
I dont mind answering the questions but would not like to disclose name of my company in public domain.
Questionaire can be redesigned and put up again taking learned member Nashbramhall's suggestions into account.

I wish to extent my sincere thanks to Nashbramhall and Nathrao . As per your guidelines i have change the personnel details, now it is not mandatory field. I have included those fields as a basic demographic requirement . Just it will be used to show my sample size nothing more to do with it.
The objective of my study is to know the existence of HR analytics at companies in
India and to know the alertness of HR analytics among the Human resource People . Practicing HR analytics in MNC companies is bit easy but is it easy for other companies to practice HR analytics.
I have not taken this topic to a highly extended level, just as a initial stage iam doing this. I wish to have your support to successfully complete my project. I here by attach the modified questionnaire for your preview.

Thank you Shirly for the response. You are still asking for their name and gender at the outset and they are marked as "Required" with a red *. I do not understand the need for that.

since i am doing this for my project i need to show some demographic elements as a proof. i may be asked to how those things. i don't know the problem behind giving there names. If you thing it as real problem let me remove the name field from mandatory one but i compulsorily need to know the gender. Thanks for your support. Hereby i attach the link.

Dear Shirly
I have taught MBAs and supervised their projects in the UK. Unless you are using the information to do some analysis and show that there is a difference between male and female responses, there is no need to collect that information. Same is the case with the other required fields about respondents.
Respondents with less than one year experience may not be know the answer to your bipolar questions so, kindly include "Do not Know" category. Then you may be able to analyse what respondents say by their experience.
