I want to do an EPF transfer from my previous company. In EPFO records, the name and address of the company are different from the current name and address. The name has been changed as it was acquired by another company. When making the transfer, which address and name should I mention in the Application Form?

It is possible that accumulation can be transferred. Simply fill Form 13 by following the process below. Transfer the Provident Fund account of a member from one establishment to another establishment covered under the act/scheme. The form is enclosed herewith for reference.

You need to submit the same after attestation by the current employer. There is no need for the current employer to approach the previous employer during the process.
