1. Why do you want to start a HR consultancy firm.
2.There are many players in the industry, what difference can you make.
3.Why should any client come to you.
4.Whats the special thing which you can give to your clients and other big players cant.
5.What kind of market survey have you done.
6.What makes you feel confident that your's can be a success story.
7.What are the all the services you want to include
8.What is your marketing plan/business plan
9. What are your short term and long term goals.
Please dont feel that i am discouraging you. I want you to be successful and thats the only reason i am trying to know if you have perfect answers for the above. There are many HR firms which just start and close within no time.
Preparing the formats is no big deal. You can prepare by yourself/search on cite hr/ seniors can provide.
Please dont start without a proper business plan. Discuss with seniors all your business plans and let them criticize. Learn from it.
If you place an ad for recruiters , there will be many people knocking your door, but if you say you want clients, no one will come to your rescue.

Before start saying my suggestion I would like to say "ALL THE VERY BEST"
As you told you have very good experience in Retail domain and it may helpful partially for your start up and other then this you should gain knowledge on Consultancy and Recruitment.
As Mr. Alex mentioned some good points and I really like to convey all those things... as well you must have confident on never give up. Because of starting consultancy is not such a easy thing in revenue generation part. After joining candidate you will get paid after 30 or 60 days and in between you have to face all kind of issues and also focus on Acquiring clients and every month onboard is most crucial part so I can say one thing you just learn the process and flow of recruitment or hire someone to deal all technical part in recruitment.
Every start up must face bottle neck situation... so before going into recruitment consultancy try to learn it completely and start with full of hope.
