how can ask a explanation letter from our staff due to unhealth job performance?. if any format?
1. Legally there is no formats such kind of issues,so prepare yourself what explanation you required from employee else inform to employee give explanation on white paper.
2. Better to take explanation from employee, motivate him in his weak areas to perform well.
3. provide needful training to him, to work harder in coming days.

As per my opinion you must talk to employee one to one.
If you ask for written document and that will demotivate & pressurized him/her in current situation.
Try to understand the person motivate him/ her, give time,if required than training for perform job.
* * there is no such format .
