Struggling with Error Code 021 on PF Portal: How Do I Fix Annexure-II Upload Issues?

Hi all, please help me with the uploading of the Annexure-II file on the PF web portal. I am facing the error code (Error Code 021) on line number 10. I have tried numerous methods to solve the issue but have not been successful. Please guide me on this matter.
