I am hoping to get a material on assessing/evaluating current performance evaluation in place in our company-- its effectivity or is it matching our current situation. Do you have a questionnaire which managers and staff can answer?
Please read some scholarly articles, as you need to find a valid and reliable instrument. And write to authors of the articles that you like, if they have not given one in their article.

It always helps experts to help you, if you give more details about yourself, the organisation (type, size, etc.) and objectives (why you are doing the survey, how many will be included in the survey, etc.). Please see <link updated to site home> and A F M Website... which may trigger more ideas.

By the way, our company is a research firm composed of Analysts who feel that our current evaluation process is subjective and tends to focus more on the recent accomplishments of the individual. The Managers who evaluate the Analysts do not undergo evaluation so the latter are proposing a 360 degree evaluation.

KPI or Key Performance Indicators are your guage on to measure the performance of the employee if he/she was able to accomplish the objectives of the department. Performance appraisals on the other hand are instruments to measure the performance of the employee.
Are KPIs the same as Key Result Areas (KRAs) and the Performance Appraisal is the method by which we evaluate the employees?
