I'm a new member on this site, although I'm a regular visitor of this website for the last 2 months. I am a 2nd-year MBA student, pursuing my winter training in Mumbai. My project title is "Measuring Post Training Effectiveness by Designing a Feedback Form." My mentor expects me to design a feedback form that consists of questions based on hard and soft skills. I have planned a few questions, but I really need help from you all.

It could be argued that each course might need its own questionnaire - certainly, there will be questions specific to a particular course that may need to be considered. I imagine you will need to define what is meant by "training effectiveness". I think you also need to define "soft skills" and "hard skills" and how you might measure changes in each. It is probable that you also need to define "value added" for the customer of the training - was it worth them sending their people on the training? Once you have these definitions clear, you should find it much easier to complete your assignment.
