Hope, u have prepared the format. The evaluation part for self appraisal should b based on Customer Satisfaction Survey, u have to prepare a questionnaire based on quality, communication, delivery, etc., where, ur immediate customer (neighbour department) rates u in 5 point rating scale. Similarly, ur superiors vl rate u on the basis of Job knowledge, Job improvements made, Personality, etc...
So u have to compare the results n recommendations and finally again rate in 5 point rating scale..........Hope this vl be useful for u.....for more queries....do post me.......Keep rockin

Thanks fo your reply.......but my question is about the method to evaluate the appraisal form what i have prepared..the form is prepared on dimensions like work output,job knowledge,interpersonal relation and few open ended questons on their acheivements..
my appraisal form has questons which are on five point rating scale and few open ended..
also tell how do u set KRA's ...
