Dear Members, I wish to inform you of the Minimum Wages for Hospital workers in Tamilnadu the Last Grade Servant is getting the minimum of Basic 9310+DA 8462=17772/- In this case, one person is appointed he is above 15000/- not eligible for PF below 21000/- eligible for ESI.
In this case, the Form 11 declaration was obtained from the Individual and declared as an excluded member of EPF and a member of ESI. This procedure is correct? Kindly clarify me, friends.

If this is his first employment with EPF then it is okay. At the same time, if he had worked somewhere and in that company he had PF, then in your company also you should give him PF. You can restrict the contribution to Rs 15000 even if the wages is above it.
For coverage of ESI the ceiling is Rs 21000 and as such he should be given ESI coverage now.

Other than what Mr. Madhu T K has been mentioned, it is essential to fill up FORM-11 for all employees who ever newly joined in the organization irrespective of cover or non cover employees under PF & Misc Act for record.
