Hi, Basically its fine, no need of employee signature as it may not be practical. You may include date of Joining and leave balance details (if applicable).

i got selected in a company with a package of 2 lacs but in hand company is giving me only 10,000 as discussed during the salary negotiation what may be the deduction as gap in package and actual getting to long , can anybody tell me.

Dear Manya,
Body of the Salary Slip: Assume the salary slip size is half page. In that put 3 columns. One for Salary Structure, another for Actual Payment Column, last for deductions column.
On the top of the 3 columns you can give the company name, address, Employee Code, Employee Name, Designation, Fathers' Name, DOJ, Employment Status etc.
Below the 3 columns Nett Pay on the right side. No.of working days, No.of days worked, No.of days leave granted,LOP on the left side.
Bottom left "Manager's Signature" and Bottom right "Employee's Signature".
I hope you can virtually bring the image in your mind of what I have described and can implement in paper.

as my package is 2 lac and company is giving only 10000 in hand , why so much gap. should i join the organisation. please suggest me , please help , is it good to join or not , waiting for help.

Ashish,
1.Why there is no ESIC applicable to you? your gross is less than 15000.
2. Annual Performance Recognition is huge, its almost 25% of your CTC. you need to consider this as at the end of the year company may tell me that you yearly performance is not up to mark hence you are not eligible for it, Make this point clear.
3. Accepting this offer is depend on how much are getting now. if you getting less than 7 to 8K then it’s ok. otherwise this is very risky.
As far as format concern, only ESIC is missing, which will further reduced your in hand by Appx Rs.750.
