Need a practical performance appraisal procedure

Hi friends,

I read this forum about performance appraisals, but here request some member to give the details about the real scenario that is being implemented in their company, so that I can understand the performance appraisal better.

Kindly provide one format as sample, and give the whole procedure about the performance appraisal process that is followed in your organisation.. .
