Hi Sundar,

You can always do that once you instill confidence among the team members that you will maintain confidentiality. You may ask them to fill out the form without mentioning their personal details. Also, you should ask them not to discuss among themselves the appraisals they have made.

Ensure that your questions are framed in a professional manner.

You can take feedback in the form of a well structured questioner. You cannot just ask subordinates to write a small note on their manager. That will amount to “work-place Politics”.
