Hi,

By the term " continuous meetings" you employer mean to say to conduct periodical meetings with the employees and to get in touch with them.. listen to their concerns / suggestions for improvement of existing HR practices etc. More importantly after the meetings implementing the corrective measures, change in process after discussing with the Top Management. There is no specific format for this activity. Probably you may maintain a document in excel or word about the feed back recd and action taken etc.
