I am working under a government department in Delhi. A few months ago, I was admitted to the hospital due to a heart attack, although this hospital is not considered under the ESIC panel. The hospital has charged almost 2 lakh rupees. When I was discharged after three days of being admitted there, I wanted to claim my medical expenses as I am a member of ESIC, and my ESIC contributions are up to date. I have contacted my local ESIC dispensary, and they have informed me that I can claim because I have an emergency certificate. The main problem I am facing now is that I am unable to find the ESIC claim form or Form B. Can you kindly provide me with a copy of the ESIC medical claim form applicable in Delhi? Thank you.

Sir, I think you or your family member can contact the ESIC dispensary to which you are attached and obtain a blank REIMBURSEMENT FORM. Furthermore, I believe you can also contact the ESIC Branch Office Manager to get the necessary guidance on the issue. These offices can also be reached by telephone, and their numbers may be available on the website of the ESI Regional/Sub Regional Offices to which your unit is attached.
