Who are the concerned persons to sign the gate pass?

To authorize someone to sign the gate pass is an internal matter of your company. Let your MD decide about it. Generally, gate passes are signed by General Managers and above. However, each gate pass has to be signed by the HOD first and then it goes to the GM for final approval.

If an employee is not informed to HR or has not obtained approval to sign the gate pass and leaves the premises, what should be done?

It appears that you have confused a Gate Pass with an Out Pass. The former is for taking material out of the company premises. There are two types of gate passes: one for returnable material and another for non-returnable material. Out passes are for people moving out of the company premises. Authorization to move out of the company, whether for personal or official reasons, is decided by the MD. Out passes are also signed by the GM. For managers, instead of out passes, a register is maintained to record their exit or entry into the company.
