Hi,

I have a query regarding PF. I quit a company in March 2015 and would like to withdraw my PF. The name on my bank account is Madhumitha S, whereas on the PF account, it is mentioned as Madhumitha Rangarajan with my father's name. I have an affidavit for the name change and an ID proof (election card) in the name Madhumitha Sharath. Along with Form 19 and Form 10C, what do I need to attach? Could anyone provide me with a declaration format for the name change?

Affidavit duly signed by a notary public stating that both the names are of one person, need to be submitted to PF Office. Pon

It is enough if the employer gives a certificate to the PF authorities along with the claim.

Dear Mr. Pon,

We have had several claims settled by the PF office based on our certificates, even in cases of a change in the name itself (like Mani and Manirathnam, and Raja and Rajagopalan, etc.). It is the rapport and relationship with the PF department officials that help - of course, some officials may insist on an affidavit.

Thank you for your reply. When I enquired with my HR, they are asking for a declaration letter from me to the PF office stating that both names are the same. If anyone can send me the letter format and what needs to be mentioned, it will be helpful.

THRE IS FORM NAMED "JOINT REQUEST" FILL THE FORM WITH BOTH EMPLOYEE AND EMPLOYER ALONG WITH IF PROOF WITH CORRECT NAME AND FORM 5 ALSO.
