We collect the following information on the feedback form: 1) Objective 2) Coverage of the topic 3) Training material feedback 4) Training infrastructure feedback 5) Trainer evaluation Currently, we do not have a standard format for reporting. If you could provide me with some guidance on creating a feedback report format, it would be a great help.
