I have been conducting Exit Interviews for the last four years and have received feedback on various odd issues - sometimes not even thought about.
One ground rule is that you should try to make the session less formal and start talking about the new job, location, and family, slowly steering your questions to the positive aspects the person experienced while being associated with the organization and whether they would consider opportunities in the near future.
After highlighting the importance of their feedback, ask them informally about what they feel needs attention or improvement in the organization. Convince them that their feedback holds great importance and will provide us with the opportunity to address any issues. This can also help in understanding the general work culture of the company and identifying areas of concern.
Lastly, we also ask the departing employee to fill out an Exit Interview form where, in addition to the informal discussion, they can write down their thoughts.
