I suggest that you kindly use a web-based questionnaire to improve the return rate. It is less time-consuming than having to download the questionnaire, answer it, save, and then send it to you.
Also, I wonder for what course this study is being conducted as most of the questions elicit only "Yes" or "No" answers.
