I need some information about the Leave with Wages Register.
1. Is it compulsory?
2. Do we need to submit it anywhere?
3. Is the frequency monthly or yearly?
4. Which type of leave do we have to count - EL/CL/SL?
5. Which period should be counted in From-To?
6. Is the name of the form Form 18?

Leave with Wages Register under the Factories Act 1948
It is covered under the Factories Act 1948.
1. Is it compulsory? Yes, it is statutory.
2. Do we need to submit it anywhere? Yes, it needs to be shown to the Inspector of Factories.
3. Is the frequency monthly or yearly? It should be shown during the Inspector's visit.
4. Which type of leave should we count, EL/CL/SL? Only earned leave should be counted, not CL or SL.
5. Which period should be counted from-to? From January to December - the calendar period.
6. Is the name of the form Form 18? No, it is Form No: 15 (Prescribed under Rules 87 and 88).
Rules for Leave with Wages Register
Rule 87: The Manager shall keep an up-to-date Register in Form No. 15 for Leave with wages.
Rule 88: The manager shall provide each worker who has become entitled to leave during a calendar year with a book in Form No. 15 not later than the 31st of January of that year.
