My previous employer had a trust with which my PF account was maintained. I am interested in the transfer of PF from the trust to the government PF office (EPFO). Could you please let me know from where to get Annexure K? Is it required when the transfer is done from a trust to the EPFO?

For your query, the PF Annexure K will be sent by the EPFO after transferring the account from one establishment to another. This is a crucial part of the PF official work. Therefore, please send Form 13 for the transfer along with essential details such as Form 3A and the member's break in service. This will enable us to receive the Annexure K and also determine the amount transferred from the old account to the present account.
