Hi Priti,
In your questionnaire, you have given objective as well as subjective questions. The format is such that these have to be filled manually. Individuals like me who don't have a printer at home cannot assist you in your research.
I would like to offer you a suggestion. Create a form that can be downloaded and filled out on the computer itself. It can then be uploaded and mailed back to you. This approach may help you receive more responses.
