I have submitted the PF form, and it was rejected due to the following reasons:

1. Father's name not mentioned in Form 3A.
2. Date of leaving mentioned as 14/03/2012 instead of 13/03/2012.

Query Regarding Non-Contributing Service Period

My query is, after correcting the date of leaving, will there be any effect on the number of days in the period of non-contributing service? If yes, how many days should be considered? I kindly request your help with this matter.

Bhavesh

Form 3A is not mandatory if your employer is filing monthly returns within the due dates. If there is any delay in PF payments by your employer, it is better to submit it along with Form 3A.

Even though Form 3A is not mandatory, it's better to enclose copies of all Form 3A. I had a bitter experience with the PF department where I received a lesser amount than I was supposed to receive. Finally, after submitting all Form 3A copies, I received the balance amount. The PF department simply stated that the company didn't provide the required documents and shifted the blame to the company, but it was actually an issue on the PF department's end.
