Procedure for Withdrawing Provident Fund After Employee's Passing

One of our employees left one year ago after 11 months of service. Subsequently, he passed away without withdrawing his provident fund. Can anyone tell me what the procedure is to withdraw his PF? What documents and information does his nominee need to withdraw the PF?

Following are the forms to be submitted:

Form – 20: In the event of the death of a member, this form is to be used by a nominee or family member to claim the member's Provident Fund accumulation along with the Death Certificate of the Member.

Form – 10D: To be submitted in duplicate by the widow/widower, orphan, or nominee, as the case may be, for claiming a pension along with the Death Certificate, and three passport-sized photographs separately for the widow and up to two children (below 25 years).

Form – 5 (I.F.): To be preferred by the person eligible to receive the Employees' Provident Fund dues of the deceased member who died while in service, along with the Death Certificate.
