i want to know that difference between contribution period and benefit period .
if an employee joins the organisation on 16 apr 2012 and wants to avail ESI benefit then wht information should be filled in form 37 asked by esi department .

Dear Naveen,
If an employee joins in the insurable employment, i.e. registered with ESI for first time, he will be provided with a temporary card, which is now available on online at the link of Print counter foil and the same is valid upto 3 months from the date of entry of such employee. On which he is entitled for Primary and Secondary medical care at Dispensary and ESI Hospital Level.
Further you asked about ESIC-37 form, which is to be required in case the Dispensary/Hospital requires it, which is a certificate given by an employer certifying either of two points; i) Whether the employee is still in insurable employment; ii) Whether he worked more than the half the no. of days or 78 days in the relevant contribution period.
With reference to Contribution period and benefit period - There are 2 contribution period in an year - 01.04 to 30.09 & 01.10 to 31.03 and relevant benefit periods are 01.07 to 31.12 & 01.01 to 30.06 respectively.
