After several discussions, ESIC authorities inserted Form 5 in their portal application Sun Java System Application Server - Server Running. The employer makes self-certification for the payments and employee details entered in the ESIC online application. I think they will ask for the Form 5 data for the submission of returns (currently, it is under process).
In the application, if the number of employees is more than 40 members, the employer will need to submit a Chartered Accountant's signature, which can be uploaded in the option of self-certification.
One good news for the ESIC insured persons. They have launched an IP portal application for insured persons. The insured person may log in with their ESIC number. The employee contributions will appear online.
