How Do You Prepare an HR MIS Report? Seeking Tips for Monthly Overviews

How is an MIS prepared in the context of HR? What if I have to prepare a monthly report that provides an overview of the entire HR activities in a particular month, such as new joiners, resignations, etc. Can anyone please email me some sample MIS?
