Hi Praveen, much thanks to you. I have gone through the address you suggested to me and obtained the following information.

Employees Provident Fund & Miscellaneous Provisions Act, 1952
Applicable to establishments engaging 20 or more employees.

- Form for registration under EPF – Form for Registration of EPF
- Business Number Form – BN_Business_No_form_of_EPF
- Form 2 - Employee nomination to EPF – Form 2 for employee nomination to EPF
- Form 3A – Contribution Cards for un-exempted establishments – Form 3A
- Form 5A – Return of Ownership to be sent to the Regional Commissioner – Form 5A
- Form 6A – Consolidated Annual Contribution Statement – Form 6A
- Form 10 – Return of Members leaving service during the month – Form 10
- Form 12A – Statement of Contribution for un-exempted establishments – Form 12-A
- Form 31 – For advance from PF – Form 31
- Form 13 – Application for transfer of EPF Account – Form 13 revised
- Form 10C – Withdrawal of PF – Form 10 C
- Form 10D - Withdrawal from Pension Fund – Form 10 D
- Form 19 – Withdrawal of provident fund – Form 19

How to Withdraw Your Provident Fund Amount (for Employees)
This is one of the most common questions employees have when leaving organizations: “How do I get back my PF amount?” It is simpler than it is usually perceived to be. The first question one needs to ask is, “Do you really want to withdraw the PF savings or simply transfer it to the new PF account with the new employer?” If the new employer does not have a PF account or if you decide to take a break, you still have two options:

Option 1 – Withdraw the PF amount using the following two forms:
- Form 19 – For withdrawal of Provident Fund Amount
- Form 10 C – For withdrawal of Pension Amount. However, if you have already contributed for more than 10 years, you need Form 10 D instead to withdraw the Pension amount.

Option 2 – Opt for a scheme certificate in lieu of withdrawal and transfer it to the PF account of your new employer you wish to join later.

If you only want to transfer your PF account to the new employer, this is a wise decision because no savings account in India offers a better interest rate than PF. Additionally, you can later opt for loans for housing, marriage, etc. If this is your decision, then you need Form 13 (Revised). Fill in the form, get your present employer's (authorized signatory) signature, and forward it to your previous employer. The PF amount in your previous employer's PF account will be transferred to your present PF account.

If you have quit your employer and are going to submit your PF withdrawal form, jot down your mobile number on top of Form 19 (PF withdrawal form). The PF department claims this will help you know the status of your claim.
