Hi, Pls guide me. Whether Form 12B is filled up by employer or employee. Previous employer is concerned about only form 16 right.

It has to be filled by the employer, not the employee. It will be issued when the employer has paid perquisites to the employee. Even if the employee doesn't have any perquisites, the employer can issue a Nil form.

The Form 12BA must be issued by the employer after remitting the TDS to the government. If required, the employer will also provide a NIL form.
You can also check through your bank account if you have registered for net banking with ICICI Bank or on the Income Tax Department website by logging in with your PAN number.
