You can get PF forms from the PF website. There is a PF withdrawal form No. 19 (attached) that you need to fill in and submit. However, before you can submit this form, you must get your PF statement either from your previous company or from their PF consultant. You also need to attach your bank details and resignation and relieving letters with the form.
Thereafter, you can get the same form signed by the HR of your previous company or by an MP or MLA and submit it to the PF office. The PF office will then process your PF withdrawal and deposit the amount directly into your bank account.

I have a situation; my employer is not providing me with a relieving letter or experience letter. He is not allowing me to meet him. So how could I withdraw my PF? I have the PF statement slips provided by the company. I have worked at that company for nearly 3 and a half years. However, he is not willing to provide me with a relieving letter or experience letter. He is demanding that I pay the full salary (36k, not just the basic salary) for 3 months, even though my notice period is only 30 days. He has pressured me to stay and threatened to file a criminal case against me, potentially ruining my life. Consequently, I felt compelled to resign voluntarily. I am unsure if my Managing Director will provide any documents for PF withdrawal. How can I handle this situation? I have around 60k in my PF account. How can I withdraw it without needing to contact the employer?

The ex-employee just needs to fill in Form No. 19, which I have attached with my earlier post. He/she should attach their latest bank statement/passbook copy with the form and give it to you. You are then required to fill in the employer's section of the form, attach their PF statement, sign and stamp the documents, and forward them to the PF office.

Gurang has already enclosed the required form. Since you worked only 3 months, you need to fill out Form 19 only, rather than Form 10C. For the withdrawal of the pension fund, the contribution period must be 180 days or 6 months. If it is less than that, no refund can be claimed.

Hi, I worked with the organization from 1st April 2010 to May 2010. Now, I would like to transfer my PF to my current organization. According to my current organization, I have to fill out Form No. 13 along with Form 3A, Form 5, and Form 10. Do you wish to check whether your PF has been transferred from one employer to another or to check whether your employer has been depositing your PF deduction with the PF office?

On a separate note, you should ideally send PF withdrawal documents by Registered AD post rather than courier. While post might take a few more days, you have a proper confirmation/receipt of delivery which the PF office can never deny.

I think it's inappropriate to reply now. Nobody wants to leave a company easily. You couldn't understand why my employer is acting like that. Same with my question also, I went so many times, and he didn't give permission to meet him. I have been working sincerely for 3+ years, and when I decided to move out, this is the reaction I received from my employer. I feel like he expected me to work for him lifelong. If the environment is so good and the employer is taking care of me so well, why would somebody want to leave from there? As long as I am a person expecting a better work environment, I couldn't find fault on my side. I did a lot of good things for the company. I have never done anything against the company. The moment I expressed my decision to leave the company, I faced such a reaction. What should I do about that? Sit over there for a lifelong? If anyone could help me in this regard, how can I get my PF withdrawal without concerning my previous employer? Each time I go there, they treat me like some previous office boy, not giving respect as a professional at least. I don't want to go there again.

There must be some reason why your employer is behaving in such a bizarre manner. If you have worked for three and a half years, surely your relations must not be so bad as to cause your employer to react like this. The best option is always to go and have a straight talk with your employer.

This, of course, requires that you have all necessary employment documents with you and also a copy of your resignation letter.
If you have all of these, then the labor court can order your previous employer to pay all dues. Also, you need not worry about the police complaint because that will automatically become of no importance if you file a labor case first.
