Hi Deepa,
I would suggest that you design a questionnaire yourself based on the various reasons why people leave and why the rest stay. The reason is that most of the time, when in an interview, you talk about the project (for fresh graduates), having designed a questionnaire leaves a good impression. There are a lot of formats available on the internet and in various books, but what's the point of following those when all you do is probably customize or simply administer the same to your population and draw conclusions.

You can prepare a questionnaire on the following points to judge the work culture and feelings of the employees:
- Respect being given
- Recognitions for the work done
- Reward scheme
- Safe working conditions
- Safety items being provided
- Health facilities
- Welfare facilities
- Recreational activities i.e. get together, company tours
- Conveyance facility
- Uniforms
- Timely payment of dues
- Mediclaim schemes
The above is not the complete list. But the points shall give you an idea to prepare a questionnaire.

Please do not request finished projects or questionnaires on this forum without giving enough information.
Instead of asking for a completed project, please search CiteHr where some projects are attached. Then do a literature survey and formulate your aim and objectives for the research (in consultation with the host organization). You also need a conceptual framework for that. Decide what hypotheses you want to test.
If you have not done any of the above essential preparation for conducting a project, I would advise you to please do so and then pose a precise question.
