Clearance form is pretty simple. Its main purpose is to declare by each department that the person has no belongings with the particular department. So the form can be created in such way that the HR dept declares that Mr/Ms xyz has resigned with effect from this date and requests each HOD to declare if s/he has any belongings in the department.
Finally, it should come to HR which has to give the final details of his number of days worked in the month (for salary payment) and ID and other documents relating to HR have been returned by the employee. Finally, the form can be sent to the accounts department for the final settlement.
