All Forms as Below:

Form for Claiming Benefits under the EPF Scheme:

Form-13 (Revised): For transferring the Provident Fund account of a member from one establishment to another establishment covered under the act/scheme.

Form-19: To be submitted by a member to withdraw his Provident Fund dues on leaving service/retirement/termination.

Form-20: In the event of the death of a member, this form is to be used by a nominee/family member to claim the member's Provident Fund accumulation.

Form-31: For the use of Provident Fund members to avail advances/withdrawals as provided in the scheme.

Form for Claiming Benefits under the Employees' Pension Scheme 1995:

Form-10C: For claiming: Refund of the Employer share, Withdrawal benefit, Scheme certificate for the retention of membership.

Form-10D: To be submitted by a member to withdraw his Provident Fund dues on leaving service/retirement/termination.

Form for Claiming Benefits under the EDLI Scheme 1976:

Form-5(IF): To be preferred by the person eligible to receive the Employees' Provident Fund dues of the deceased member who died while in services.

One-time Returns on Coverage:

Form-1 (Revised): This is given by PF members and utilized for releasing PF dues to the nominees. It also contains the details of the family members.

Form-5A: Return of ownership to be sent to the Regional Commissioner (in duplicate) within 15 days of coverage.

Form-9: Return of employees who are entitled and required to become members of the Employees' Provident Fund and Pension Fund (within 15 days of coverage).

Monthly:

Form-5: Return of Employees qualifying for membership to the Employees' Provident Fund for the first time during every month within 15 days of the following month.

Form-10: Return of members leaving service during the month.

Form-12A: Consolidated Statement of dues and remittance by the 25th of the following month to which the dues relate.

Annual:

Form-3A: Member's annual Contribution card: Form showing month-wise recoveries towards E.P.F and Pension Fund in respect of a member for one financial year. To be furnished by the employer before the 30th of April of the following year.

Form-6A: Consolidated annual contribution statement: This form provides annual contributions of each member of the establishment. A vital form for compiling the annual Provident Fund statement of a subscriber. To be submitted by the 30th of April.

Monthly Pension Purpose:

Form-F4(PS): Within 15 days of the close of every month.

Form-F5(PS): Return of Members Leaving Service During the Month within 15 days of the close of every month.

Hi,

In my company, there are 7 employees. I joined the company in September 2009. The company is deducting my PF employer contribution without PF registration. So, my question is, will the company provide me with my PF employer contribution from September 2009 to March 2010?

Hi Yamuna,

Here are some benefits of the PF Act:

Members will receive a pension upon superannuation or retirement from service and in case of disablement during employment. Family pension will be provided to a widow or widower for life or until remarriage. Additionally, children are entitled to pension up to the age of 25. If there are orphans, they can receive a pension at an enhanced rate upon the death of the widow or widower, or if the widow pension ceases. The pension benefit for children or orphans is limited to two children or orphans. Any withdrawal benefits, etc.
