Dear Prabhakaran, thank you for clearly stating the objectives of your study, unlike most students who seek questionnaires with one-line requests. Kindly try to avoid the use of SMS language. Please search the web using keywords from your secondary objectives and read scholarly articles to see what others have done, how, and why they did it that way. For example, http://www.emeraldinsight.com/journa...;show=abstract and www.emeraldinsight.com list all the journals published by them, and people can access abstracts for free. The authors may be willing to provide a copy if you write to them. Then, you may find a few questionnaires that you may have to combine to suit your needs.
