You have to prepare a DD for LWF on behalf of the "Labour Welfare Fund Commissioner" and you need to fill up a form for it, which is available at the local Labour office. The contribution you have to pay before the 30th of December every year.

Normally, the contribution is to be made by cheque, drawn in favor of 'The Secretary, State Labour Welfare Board,' and is to be sent to the state Labour Welfare Board situated in your area/city. The employer contribution is 16% or Rs 500, whichever is the larger amount, and employee contribution is 8%.
The mode of payment is by cheque or DD. I suggest you stay in touch with your state's Labour Department for notifications and details.
