The individual assessment I mean is preparing a format in which we can ask our employees some questions. For example:
1) Are they happy with their current job profile?
2) How faithful are they to the company, indicating a percentage?
3) Special achievements in the current year.
4) Providing a rating to their colleagues between 1 to 10.
And we can add more questions to it...
So, does anybody have the format? Please forward it. Did you get my idea?

In a clearer sense, performance denotes an employee's ability to utilize their talent.
In any employee appraisal, both aspects need to be covered. Through potential appraisal, you can understand where the employee's strengths lie and where they may require counseling or improvement. Subsequently, monitoring the assessment of their performance becomes more manageable.
