The dictionary meaning of the word "affidavit" is "written declaration made under oath", whereas the dictionary meaning of the word "indemnity" is "protection against future loss". By correcting your name whom will you protect from future loss? Therefore, you need to submit an affidavit and not an indemnity bond.
While writing the affidavit, write in sequence, who you are, what is your UAN number, what inaccurate name is entered in the EPFO, what should have been the correct name and what document you are providing in support of the correct name (like Class X marklist or certificate), etc.
