“behavioral based self assessment sheet for workers”
The above is a very vague term
Is it for yearly / half yearly overall performance in line with goal set?
Is it for safe working and other connected activities?
Is it for quality of the product produced in the line?
Attitude, culture and behavior influences one another and behavior is the one we can observe. For Contributors to business, Attitude, Behavior, and Culture differ from one group to others, because they represent intangibles that cannot be shifted with conversation alone. Individuals must take it upon themselves to change. What we consider right and wrong. This is how the society we live in influences our choices. But our choices can also influence others and ultimately help shape our society.
My personal opinion, we may not be successful, and can have behavior based self assessment. In India, culture / attitude shapes the way the worker’s performance / perception, and it makes a difference in how they view themselves compared with others. It affects their values—what we consider right and wrong, may not be acceptable to all. This is how we manage work force as they are, and they live in influences our Management culture / choices, they chose to adopt. A conscious effort / continuous process is required from top to bottom of any organisation structure, for a change required. We are now managing with different combination of attitudes, culture and behaviors in a manufacturing process.
My request would be, If at all you could get one/ develop one, I would request you to please share for my learning, understanding.

I'm not really sure developing this yourself is a good idea. Sounds like it is something that you need to contract a professional to provide.
Why are you looking at self-assement as opposed to doing regular performance evaluations, and training needs assessments?

Dear madam,
Unless one knows the purpose and end results to be achieved in designing behavioural self assessment for workers, it is not possible to add value.
There also has to be clarity in what 'behaviour' is sought to be self assessed - is it work related or general- and whether workers have capability to self assess.
Some members on this forum can surely make significant contribution ,provided basic conceptual clarity on above points is available.
