Yes, you will need to submit any 3 to 4 documents as proof (Aadhar, PAN, mark sheets, Voter ID, DL) along with the Joint Declaration Form to the EPFO Office.
Alternatively, you can upload the same documents in the Basic Details Change request on the EPFO Member portal.
Once your father's name is changed, you can apply for your transfer request.
