I would like to know who conducts the police verification. Does an employee have to get it done by himself/herself by visiting the nearby police station, or is it taken care of by the employer?

This is the prerogative of the employer.
Generally, in government services, applicants submit a character certificate. However, based on my experience, in the private sector, it is conducted through a third party appointed by the employer. The employer usually requires a separate affidavit or information provided in the application form regarding any involvement as a culprit in a civil or criminal matter.
