Hi guys, we have lost our PF's allotment letter in a fire accident recently. I want to reissue the PF's allotment letter once again from the PF department. So, I want to know if there is any format for this letter to be reissued. If you have one, kindly share it with me as soon as possible. I would appreciate this help.

Firstly, you should approach the concerned EPFO with the following documents and request them to reissue the duplicate:
1. A covering letter explaining the situation regarding the loss.
2. Copy of Form-5 (Return of Ownership) in duplicate.
3. List of the Board of Directors with their corresponding addresses.
4. Memorandum of the Articles of Association.
5. Bank details.
6. Latest paid EPFO challan (Photocopy).
7. Report from the Police/Fire Department.
All the aforementioned documents must be attested by the owner/authorized signatory and submitted to the Regional PF Commissioner at the concerned EPFO, duly acknowledging the same.
