Registers to Maintain in a Chemical Manufacturing Industry
Refer to your query in any manufacturing industry, including the chemical industry, you have to maintain the following registers under statutory provisions laid down as per Labour Enactments.
1. Factories Act:
- Form 11 - Notice of Periods of Work
- Form 25 - Muster Roll
- Form 10 - Register of OT
- Form 15 - Register of Annual Leave with wages
- Form 12 - Adult Register
- Form 26 - Register of Accidents causing Death or Bodily Injury
- Form 18 - Notice of Accident for Fatal Accidents
- Form 18(1) - Notice of Accident for Non-Fatal Accidents
- Form 5 - Certificate of Fitness issued by Certifying Surgeon after Medical Examination of employees
- Form 8 - Certificate issued by a competent Person for Pressure Vessels/CPB's
- Form 33 - Certificate issued by a competent person for Lifting Tools/Tackles/Hoists, etc.
- Common Annual Return (Form 22)
2. Contract Labour (R & A) Act - 1970:
- To obtain Registration Certificate
- Form IV - Register of Contractors
- Annual Returns
3. EPF MISC. Provisions Act -1952:
- To obtain a Code Number by submission of Form 1 along with principal employer particulars
- Form 2 (A) - Revised - Nomination and Declaration Form
- Form 5 & 10 - New & Left employees particulars
- Form 12 (A) - Revised - Monthly Return
- Combined Challan under A/c No. 1, 2, 10, 21 & 22
- Form 3 A & 6 A - Annual Return
- Form 19 & 10 C - For withdrawal of PF Amount
- Form 13 (R) - Transfer of PF Accumulations
- Form 10 D - For Pension
- Form 20 - To receive the PF Amount by the Nominee/Legal Heir in the case of Death
- Form 5 (IF) - For EDLI Benefit
- Form 31 - For Loans/Advances

- To obtain a Code Number by submission of Form 1
- Form 1 & 3 in Duplicate - Declaration Form and details of employees list
- Form 1 (B) - Additions/Deletions

- Form 6 - Half Yearly Returns
- Form 12 - Notice of Accident
- Form 15 - Accident Register
- Form 37 - Certificate of Employment
- Form 52 - Change of Dispensary/Local Office
- Form 71 - Issue of Duplicate ID Card
- Form 86 - Certificate of employment
- Form 105 - Certificate issued by the employer to avail Temporary Treatment to IP when he was out of station
5. Payment of Bonus Act - 1965: Form A, B, C, D Registers
6. Payment of Gratuity Act -1971:
- Nomination and Declaration Form
- Change of Particulars of Nominee
- Notice submitted to the employer by the employee for payment of Gratuity
- Intimation sent by the employer to the employee to receive Payment of Gratuity
7. Industrial Dispute Act - 1947:
- Form P & PA - For Retrenchment
- Form Q & QA - For Closure
8. Indl. Employment (Standing Orders) Act - 1947:
- Submission of Draft copies of Standing Orders for certification
