Dear Sir, One of my employees has been on maternity leave for 3 months (as per the ESI Act). She is eligible for ESI leave and has returned to work after completing her ESI leaves. The ESI office is requesting a letter confirming her return to duties after maternity leave before processing her claim for the ESI amount. Providing this letter from the employer's side is necessary to initiate further processing of her claim. Could you please provide a letter format that we can submit to the ESI office to facilitate her maternity amount claim?

Documentation for Maternity Benefits and Sickness

You have to give a joining report in Form 12A, 13, 14, 15 for maternity benefits and sickness as per the ESI Act of 1948. Did you ask her to register her ESI card before she went for maternity leave? Did she get the doctor's certificate for maternity and the discharge certificate from the hospital?

Please ensure that all necessary documentation is completed and submitted according to the regulations.

Usually, the ESI authority will provide the confidential form to the employer for signing regarding the leave details. There may not be a separate letter from the employer. Please ask your employee to obtain the form from the local ESI office.
