Sir,
My mother is seriously ill (or suffering from ___________________)and stays put up in my hometown/village and being the elder/eldest son/daughter (or) the others are away (or) legally separated (or) not in a position to look ater her.
The present condition has become a mental agony for me and I am unable to contribute to the organization to the fullest of my satisfaction, as such I have decided to stay at home and take care of my mother and fulfil my social obligation/responsibility.
Kindly accept my resignation and honour a relieving at the earliest.
